Visco-supplementation in osteoarthritis

Sinovial is a 0.8% physiological solution of sodium hyaluronate (HA) in sterile 2 ml-prefilled syringe for intra-articular injections.

The active ingredient of Sinovial is obtained by biofermentation and high purification processes ensuring a product with a defined molecular weight (1 Mio D). It is totally free from the animal proteins occurring in the extractive materials, thus exerting improved immunological and viral safety.

The ideal balance of MW to concentration guarantees an optimal injectability with increased chances of correct needle positioning as well as reduced pain at injection.

Sinovial is indicated for the replacement and complementing of synovial fluid in case of diseases of the articulation of degenerative or traumatic origin. The administration of Sinovial results in a marked pain reduction and improvement of the articular mobility. The effects appear already after the first injection (out of a cycle of 3 to 5) and last up to 6 months (remnant effect).

Visco-induction in osteoarthritis

The discrepancy between the long-lasting (months) clinical efficacy compared with the relatively short (days) half-life of intra-articular hyaluronic acid may be explained by its CD44-mediated metabolic modulation. This results in anti-inflammatory and analgesic properties and in the restoration of the natural synthesis of hyaluronic acid by the synovial cells. This has been referred to as visco-induction to mark the difference with the old concept of visco-supplementation. The clinical value of the metabolic effects of HA may by far exceed that of the lubricant effect and is the most modern explanation of the mechanism of action of HA.

The medium molecular weight (1 Mio D) of Sinovial has been described as the best for interation with and activation of the receptor CD44, which confers to Sinovial visco-inductive properties that may not occur with preparates of different molecular weights.
